Medicaid is a state-administered, publicly funded health insurance program supported jointly by federal and state governments. It serves low-income families and individuals, children, seniors, and people with disabilities and is among the core components of the U.S. health care system.

Because taxpayer funds pay for Medicaid, fluctuations in local billing help illustrate how a community directs its public health care resources.

The “Vision Services” category encompasses a range of Medicaid-billed services grouped by care type, using standardized HCPCS and CPT code ranges. For this report, each code was assigned to only one service category based on consistent prefixes and numbers, ensuring accurate year-to-year rankings and preventing any double counting of services.

Medicaid expenditures climbed in several major categories, but Vision Services ranked eighth overall in Selah in terms of Medicaid billing for 2024.

At the state level, Vision Services held the 20th position for total Medicaid payments in Washington in 2024.

From 2020 through 2024, Vision Services-related Medicaid payments in Selah grew by $861, or 0%. The pace of spending picked up especially during select periods, with year-over-year upticks notably in 2023 and 2022.

Though spending was distributed citywide, a small number of ZIP codes accounted for the bulk of Medicaid values in Vision Services. In 2024, ZIP code 98942 stood out with a total of $861, making up 100% of Medicaid payments in this category in the city for the year.

A limited set of billing codes within the Vision Services category comprised the majority of Medicaid payments during this span.

To compare, the 202.1% boost in Vision Services Medicaid payments in Selah from 2023 to 2024 outpaced the overall growth across all Medicaid claim categories in the city during the same time, which was 28.4%.

According to the Centers for Medicare & Medicaid Services, federal and state spending under Medicaid reached approximately $871.7 billion in fiscal year 2023, or about 18% of all U.S. health expenditures. This is a sharp rise from $613.5 billion in 2019, the year before the COVID-19 pandemic.

This shift reflects growth of nearly 40% over several years, largely due to expanded Medicaid enrollment and increased service use during and after the pandemic.

The Trump administration passed budget legislation featuring substantial reductions in federal Medicaid funding and changes to the program’s structure. Specifically, the “One Big Beautiful Bill Act,” became law in 2025 and is forecasted to cut more than $1 trillion from federal Medicaid spending over 10 years. It puts in place elements like work requirements and higher cost-sharing, which could limit coverage and reduce funding for certain enrollees. Consequently, more of the financial burden may shift to states, slowing the expansion of federal Medicaid aid even as program enrollment remains high.
